Status: C = Certified, P = Provisionally Certified, R = To be revoked on the effective date shown. Provisional certification means that the Department deems the laboratory capable of producing valid data despite minor deficiencies. The Department may accept results from laboratories that are certified or provisionally certified by the Department.
